From consultation transcript to draft clinical documentation
NexusMed helps clinicians turn a stored consultation transcript into a draft clinical record and patient letter. These are materials for review and editing. Clinical decisions and final wording remain with the clinician.
Updated:
Start with the source material
The stored consultation text is the source for the document. Clinicians can add their notes and select an available template. Before first use, agree recording conditions, data scope and the required document format. A de-identified sample is sufficient for an initial demonstration.
- Agree how the consultation will be recorded and processed.
- Check that important parts of the conversation appear in the transcript.
- Add observations or clarifications that were not spoken.
Prepare and review the record
After the consultation ends, NexusMed uses the stored transcript and clinician notes to draft a structured record. The selected template determines sections and fields. Review and edit the result before saving. Information absent from the source does not become established fact simply because it appears in an AI draft.
- Compare symptoms and history with the source conversation.
- Check dates, numbers, medicine names and dosages.
- Remove unsupported statements and complete missing information.
A patient letter serves a different purpose
The clinical record organizes information for the clinician’s documentation. A patient letter explains consultation outcomes and next steps in more accessible language. NexusMed prepares a draft; the clinician checks prescriptions, wording and recipient details, then decides how to share it. Sending without review is not assumed.
Understand the limitations
Audio quality, transcript completeness, terminology, notes and the selected template affect the draft. Both transcription and generation can make mistakes. If source text changes after generation, consider whether a new draft is needed. No fixed accuracy percentage or guaranteed time saving is claimed. Evaluate total preparation time, including review and corrections, using representative consultations.
Evaluate your document format
Compare one de-identified example with your clinic’s template: what is transferred, what still needs manual entry and what the final revision looks like. Confirm the availability of a specific template in a demonstration. Drafting does not imply signing, legal validation or automated regulatory submission. Some available templates relate to Russian clinical workflows; this English page does not establish availability in a particular country.
- Choose a representative sample and list required fields.
- Review the output with a clinician who handles that visit type.
- If EHR transfer is needed, check the specific form and save workflow.
